Flash Performance, 6609, #103, Royal St, Pleasant Valley, MO 64068

$5 per player

 

Payments must be made online due to limited number of players allowed.

 

Limited number of athletes in accordance to state and local regulations.

 

At this time athletes and coaches must enter and exit thru the back door and agree to the following guidelines:

  • If the athlete or parent is sick, please do not come to the facility. This includes but not limited to fever, cough, shortness of breath and other symptoms of COVID-19 outlined by the CDC.
  • Athletes and coaches may arrive no more than 10 minutes before the activity and must be picked up/leave immediately after activity concludes.
  • Temperature checks will be required before athlete enters the facility.
  • Coaches and athletes will wear a mask when entering and exiting facility.
  • All players and coaches must sanitize their hands prior to entering the gym area and after leaving the court.
  • There will be a maximum of 16 players on the court.
  • No spectators allowed in the facility during activity at this time.
  • There will be no tables or chairs available to put equipment on.
  • Athletes and coaches will bring their own water.  Water fountains will not be open.
  • Hand sanitizer will be available however, it is strongly suggested that athletes and coaches bring their own

INJURY AND COVID - 19 WAIVER

The undersigned states that he/she understands that Kansas City Volleyball Club LLC is not and shall not be responsible for or liable for any illness, or injury to person or damage to property resulting from the program which the undersigned is registering or being registered or from his/her participating in said program, and the participant and the undersigned, if the participant is a minor, herby forever releases and holds harmless the said Kansas City Volleyball Club LLC. , its employees, agents and representatives from any and all claims of any kind that the participants may have or claim to have resulting from participating in said program. I have read and understand the waiver statement.

 

COVID – 19

The health and well-being of our staff and athletes remains our top priority. In order to minimize the risk of COVID-19 entering our environment and spreading amongst our community, we will only allow staff assigned to work and athletes scheduled to participate to be in our gym at this time.

Kansas City Volleyball Club (hereafter referred to as the “Club”) has put in place numerous preventative measures and enhanced cleaning protocols to reduce the likelihood of spreading COVID-19 in the gym environment; however, the Club cannot guarantee that you or your child(ren) will not become infected with COVID-19. Further, attending sponsored activities at the Club could increase your risk and your child(ren)’s risk of contracting COVID-19.

By signing this agreement, I acknowledge the contagious nature of COVID-19 and voluntarily agree to the participation terms described above and assume the risk that my child(ren) and I may be exposed to or infected by COVID-19 by attending the Club and that such exposure or infection may result in personal injury, illness, permanent disability, and death.

All participants in Kansas City Club Volleyball’s sponsored activities agree and abide by the following:

  • Athletes and coaches will bring their own water.  Water fountains will not be open.
  • Hand sanitizer will be available however, it is strongly suggested that athletes and coaches bring their own.

I understand that the risk of becoming exposed to or infected by COVID-19 at the Club may result from the actions, omissions, or negligence of myself and others, including, but not limited to, Club employees, volunteers, and program participants and their families. I voluntarily agree to assume all of the foregoing risks and accept sole responsibility for any injury to my child(ren) or myself (including, but not limited to, personal injury, disability, and death), illness, damage, loss, claim, liability, or expense, of any kind, that I or my child(ren) may experience or incur in connection with my child(ren)’s attendance at the Club or participation in Club programming (“Claims”).

I further acknowledge and agree that, due to the nature of how volleyball is played, social distancing of 6 feet per person may not be possible at all times. I fully understand and appreciate both the known and potential dangers of performing this activity, and acknowledge that, despite Kansas City Volleyball Club’s reasonable efforts to mitigate such dangers, this activity may result in exposure to COVID-19, which could result in quarantine requirements, serious illness, disability, and/or death. I agree that, by participating in this contracted engagement, and by accepting the consideration received, I am expressly assuming the risk of such participation for me and/or my son/daughter.

On my behalf, and on behalf of my children, I hereby release, covenant not to sue, discharge, and hold harmless the Club, its employees, agents, and representatives, of and from the Claims, including all liabilities, claims, actions, damages, costs or expenses of any kind arising out of or relating thereto.

I understand and agree that this release includes any Claims based on the actions, omissions, or negligence of the Club, its employees, agents, and representatives, whether a COVID-19 infection occurs before, during, or after participation in any Club activity.

 

Flash Performance Waiver

COVID-19 is a new illness that can affect your lungs and airways. It’s caused by a virus called coronavirus. In order to stop or slow the spread of the virus Missouri has issued an Executive Order declaring a Sate of Emergency. Although we will endeavor to take all reasonable precautions including as to the cleaning and disinfection of surfaces, there is still a risk that you may be exposed to the COVID- 19 virus. 

Your signature below acknowledges your understanding of this risk and your agreement to take all precautions as recommended by the Centers for Disease Control and Prevention, State of Missouri Department of Health and Senior Services and other agencies and as required by this facility and our staff. These precautions include: 

-Wash hands with soap and water for at least 20 seconds. 

-Cover your cough or sneeze with a tissue of your arm. 

-Use alcohol-based hand sanitizer frequently. 

-Stay 6 feet apart from other people. 

-Don’t shake hands. 

-Don’t touch any part of your face. 

By signing below, I acknowledge that I have read the foregoing and agree to abide by all applicable precautions and facility directives and consent to admission to this facility. I further release this facility and its owners, agents and assigns from any liabilities and any and all claims related to my exposure to or contracting of an illness related to the COVID-19 virus while residing at or being present in this facility.
